White Wolf.  It's part of their Vampire: The Masquarade series (I butchered that word, I know, it's late)

You mean when did the guy debut or the character.  Undertaker the character I believe was the summerslam 90 (it hink it was 90)and I think Mark Calloway debuted in like 1989

The ones that have been missed (that I know)

2. The way he threw those fireballs around, I'm surprised there's not more than one. To be honest, I'm not sure but the guy had a long running feud with Bobo Brazil, so I'll play the odds and say him.

4. I THINK it might be Dick Slater. I remember some sort of bounty angle involving him, Orton and Flair where Slater and Orton teamed up, so it's natural to think the answer is Slater, but that's just an educated guess.

7. As Kane, I presume. Now, his first televised and advertised match was against Mankind, but he had already fought Vader on the house show circuit a few times - that ties in quite well with the "really fucked up" part of the question too, as Kane smashed Vaders face in several months later with an overly sized wrench.
