blank foreign object

I fairly frequently run across this small problem in matches involving foreign objects as pretty much all of mine do. The match text will read something like:
[wrestler1] hits [wrestler2] with .
It doesn't seem particular to any particular wrestler or type of match.  I've looked several times through the foreign object database and all the articles (a, an, the) are there. Has anyone else encountered this and found a solution?
I suppose I need to start logging occurences of this to hopefully track down the source by elimination.

Actually, it would be best if you could provide snippets of commentary with what happened right before this happens and what happened right afterwards. That might be able to steer me in the right direction.

This has been reported on and off for years, however I never found the source of the problem nor the circumstances in which it happens.
